First Visit

Patients need to bring:

• Your completed Patient Information Form & Medical History (attached)
• Referral letter from your GP or Specialist
• Medicare card and Private Insurance card
• Any relevant recent x-rays or films you may have, if you are from out of state please ensure you have a hard copy of your films, as we cannot access the films online.
• Military paperwork for defence personnel
• If you are a female seeing Dr Damiani as a shoulder patient please wear a camisole to your appointment to facilitate the examination of your shoulder
